Evenings and weekends are included at no extra cost. We proudly serve patients within a 50-mile radius of Boulder, Colorado, including Denver, Greeley, Fort Collins, Estes Park, and surrounding areas. After your visit, you will receive a detailed receipt for optional insurance reimbursement submission.

A dedicated nurse practitioner comes directly to you for a full medical visit that includes:

• Comprehensive physical exam

• Necessary labs

• Medication refills

⭐⭐ Concierge Membership: Greater Access and Unmatched Convenience!

With our Concierge Membership, you receive two monthly home visits and unlimited telehealth visits. The full Concierge Primary Care Membership is just $100 per month per individual, under a 6-month contract. You will also receive a detailed receipt for optional insurance reimbursement submission.
