Concierge Care Program

Our Concierge Care Program delivers a seamless, accessible healthcare experience—at home, at work, or wherever you need support. Choose a one-time visit or become a member for ongoing personalized care.  We offer 2 levels of concierge service to our patients.

⭐ Single Home Visit:  $150 - Evenings and weekends included at no extra cost. We proudly serve patients within a 50-mile radius of Boulder, Colorado. Including Denver, Greeley, Fort Collins, Estes Park, and surrounding areas. You will also receive a detailed receipt for optional insurance reimbursement submission.

A dedicated nurse practitioner comes directly to you for a full medical visit that includes:

      •Comprehensive physical exam

      •Necessary labs

      •Medication refills

      •Diagnosis & treatment of your current concerns.

⭐⭐Concierge Membership: Your Own Personal Provider — Greater Access and Unmatched Convenience!

Two monthly home visits and Unlimited Telehealth Visits. Perfect for anyone who values accessibility, holistic support, and a long-term health partnership.  Full Concierge Primary Care Membership are $100 per month per individual, under a 6-month contract.  You will also receive a detailed receipt for optional insurance reimbursement submission.

Members receive:

      •Direct provider communication (text, call, or portal)

      •Longer, unhurried appointment times

      •Annual comprehensive wellness planning

      •Priority access for urgent issues

      •Medication management & refills

      •Routine labs included

      •Unlimited telehealth visits

      •Two in-home visits every month (within 50 miles)
